A collaborative interprofessional team of faculty and partners from the IU School of Nursing, IU School of Medicine, and IU Health Bloomington received a National Academies of Practice (NAP) Creativity in Practice and Education Award for their Navigator Project. The award was presented at the NAP Annual Conference on March 18, 2017, in Philadelphia, PA. During the Navigator Project, senior nursing and second-year medical students worked in teams to make home visits to patients at high risk for readmission. Results showed a reduction in 30-day readmission, a reduction in all readmissions, and increased days to readmission. Faculty members and nurses leading the project are
